Anatomical Position and Directional Terms

Standard Anatomical Position

The anatomical position is a universally accepted reference for describing the location and relation of body parts. In this position, the body stands erect, feet slightly apart, palms facing forward, and thumbs pointing away from the body. All directional terms are based on this standard position, ensuring consistency in anatomical descriptions.

Directional Terms

Directional terms are used to describe the location of one body structure relative to another. These terms are always referenced from the standard anatomical position, not the observer's perspective.

  • Superior (cranial): Toward the head or upper part of a structure or the body; above.

  • Inferior (caudal): Away from the head or toward the lower part of a structure or the body; below.

  • Anterior (ventral): Toward or at the front of the body; in front of.

  • Posterior (dorsal): Toward or at the back of the body; behind.

  • Medial: Toward or at the midline of the body; on the inner side.

  • Lateral: Away from the midline of the body; on the outer side.

  • Intermediate: Between a more medial and a more lateral structure.

Regional Terms

Major Divisions of the Body

The human body is divided into two major regions: the axial and appendicular regions. Regional terms specify particular areas within these divisions.

  • Axial: Includes the head, neck, and trunk.

  • Appendicular: Includes the limbs (arms and legs).

Body Planes and Sections

Body Planes

Body planes are imaginary lines along which the body or its parts may be cut for anatomical study. The three most common planes are:

  • Sagittal plane: Divides the body vertically into right and left parts.

  • Midsagittal (median) plane: A sagittal plane that lies exactly in the midline.

  • Parasagittal plane: A sagittal plane that is offset from the midline.

  • Frontal (coronal) plane: Divides the body vertically into anterior (front) and posterior (back) parts.

  • Transverse (horizontal) plane: Divides the body horizontally into superior (top) and inferior (bottom) parts.

  • Oblique section: A cut made at an angle other than 90° to the vertical plane.

Body Cavities and Membranes

Major Body Cavities

The body contains internal cavities that are closed to the environment, providing protection to organs. There are two main sets of cavities:

  • Dorsal body cavity: Protects the nervous system and has two subdivisions:

    • Cranial cavity: Encases the brain.

    • Vertebral cavity: Encases the spinal cord.

  • Ventral body cavity: Houses internal organs (viscera) and is divided by the diaphragm into:

    • Thoracic cavity: Contains pleural cavities (lungs), mediastinum (pericardial cavity and other thoracic organs), and pericardial cavity (heart).

    • Abdominopelvic cavity: Contains abdominal cavity (stomach, intestines, spleen, liver) and pelvic cavity (urinary bladder, reproductive organs, rectum).

Serous membranes (serosa) are thin, double-layered membranes covering surfaces in the ventral body cavity. The parietal serosa lines internal cavity walls, while the visceral serosa covers internal organs. The space between these layers is filled with serous fluid, reducing friction.

  • Pericardium: Associated with the heart.

  • Pleurae: Associated with the lungs.

  • Peritoneum: Associated with the abdominopelvic cavity.

Abdominopelvic Quadrants

Medical personnel often use quadrants to describe locations in the abdominopelvic region. The region is divided into four quadrants:

  • Right upper quadrant (RUQ)

  • Left upper quadrant (LUQ)

  • Right lower quadrant (RLQ)

  • Left lower quadrant (LLQ)

Anatomists use nine regions, resembling a tic-tac-toe grid, for more precise localization:

  • Right hypochondriac region

  • Epigastric region

  • Left hypochondriac region

  • Hypogastric region

Besides the main dorsal and ventral cavities, the body has several smaller cavities:

  • Exposed to environment: Oral and digestive cavities, nasal cavity, orbital cavities, middle ear cavities.

  • Not exposed to environment: Synovial cavities (joint cavities).

Homeostatic Imbalances

  • Hiatal hernia: Occurs when part of the stomach protrudes through the diaphragm into the thoracic cavity, potentially causing heartburn.

  • Pleurisy and peritonitis: Inflammation of serous membranes can cause pain and complications.

  • Wrong site surgery: Critical errors due to confusion about anatomical terminology highlight the importance of mastering anatomical terms.
