Introduction
The digestive system is essential for breaking down food into nutrients, absorbing these nutrients into the bloodstream, and eliminating indigestible remains. Understanding its structure and function is crucial for assessing and treating digestive disorders.
Main Functions of the Digestive System
Ingestion: Taking in food through the mouth.
Mechanical Breakdown: Chewing, mixing, and churning food to increase surface area for enzymes.
Propulsion: Moving food through the alimentary canal via swallowing and peristalsis.
Digestion: Enzymatic breakdown of complex food molecules into simpler chemical building blocks.
Absorption: Transport of digested nutrients from the GI tract into blood or lymph.
Defecation: Elimination of indigestible substances as feces.
Organs of the Digestive System
Alimentary Canal (GI Tract): A continuous muscular tube from mouth to anus, including the mouth, pharynx, esophagus, stomach, small intestine, large intestine, and anus. Responsible for digestion and absorption.
Accessory Digestive Organs: Teeth, tongue, gallbladder, salivary glands, liver, and pancreas. These organs assist in the breakdown of food.

Organization of the Digestive System
Relationship to the Peritoneum
Peritoneum: Serous membrane of the abdominal cavity, consisting of the visceral peritoneum (covers organs) and parietal peritoneum (lines body wall).
Peritoneal Cavity: Fluid-filled space between the two peritoneums, lubricating mobile organs.
Mesentery: Double layer of peritoneum that supports digestive organs, provides routes for blood vessels, lymphatics, and nerves, and stores fat.
Intraperitoneal Organs: Located within the peritoneum.
Retroperitoneal Organs: Located outside or posterior to the peritoneum (e.g., most of the pancreas, duodenum, parts of the large intestine).

Clinical Note: Peritonitis
Inflammation of the peritoneum, often due to infection or injury.
Can be life-threatening if widespread; treated with antibiotics and surgical removal of debris.
Histology of the Alimentary Canal
Four Basic Layers (Tunics)
Mucosa: Innermost layer; secretes mucus, digestive enzymes, and hormones; absorbs nutrients; protects against infection. Composed of epithelium, lamina propria, and muscularis mucosae.
Submucosa: Areolar connective tissue with blood and lymphatic vessels, lymphoid follicles, and nerve plexuses.
Muscularis Externa: Responsible for segmentation and peristalsis; consists of inner circular and outer longitudinal muscle layers. Sphincters are formed by thickened circular layers.
Serosa: Outermost layer; visceral peritoneum. In the esophagus, replaced by fibrous adventitia.

Blood Supply: Splanchnic Circulation
Includes arteries branching from the aorta (hepatic, splenic, left gastric, inferior and superior mesenteric arteries).
Hepatic Portal Circulation: Drains nutrient-rich blood from digestive organs and delivers it to the liver for processing.
Control of Digestive System
Enteric Nervous System (ENS)
The GI tract has its own nervous system, the enteric nervous system (ENS), also called the "gut brain." It contains more neurons than the spinal cord and regulates GI motility and secretion.
Submucosal Nerve Plexus: Regulates glands and smooth muscle in the mucosa.
Myenteric Nerve Plexus: Controls GI tract motility.

Neural Reflex Pathways
Short Reflexes: Mediated by the ENS; respond to stimuli within the GI tract.
Long Reflexes: Involve the central nervous system; respond to stimuli inside or outside the GI tract.
Parasympathetic Stimulation: Enhances digestive activity.
Sympathetic Stimulation: Inhibits digestive activity.

Mouth and Associated Organs
Mouth (Oral Cavity)
Bounded by lips, cheeks, palate, and tongue.
Lined with stratified squamous epithelium for protection.
Palate forms the roof of the mouth, divided into hard (bony) and soft (muscular) palate.
Uvula: Fingerlike projection from the soft palate.

Tongue
Composed of skeletal muscle; manipulates food, forms the bolus, and initiates swallowing and speech.
Surface features papillae: filiform (friction), fungiform (taste), vallate (taste), and foliate (taste).
Lingual frenulum anchors the tongue to the floor of the mouth.
Terminal sulcus divides the oral and pharyngeal parts of the tongue.

Salivary Glands
Produce saliva, which cleanses the mouth, dissolves food, moistens food, and begins starch digestion with amylase.
Major glands: parotid, submandibular, and sublingual.
Saliva contains water, electrolytes, enzymes (amylase, lingual lipase), proteins (mucin, lysozyme, IgA), and metabolic wastes.
Control: Parasympathetic stimulation increases secretion; sympathetic stimulation decreases it (dry mouth).

The Teeth
Dentition and Dental Formula
Primary (deciduous) teeth: 20, erupt between 6–24 months.
Permanent teeth: 32, replace primary teeth between ages 6–12.
Types: incisors (cutting), canines (tearing), premolars (grinding), molars (crushing).

Tooth Structure
Crown: Exposed part above the gum, covered by enamel (hardest substance in the body).
Root: Embedded in the jawbone, attached by the periodontal ligament.
Dentin: Bonelike material under enamel, maintained by odontoblasts.
Pulp Cavity: Contains connective tissue, blood vessels, and nerves.
Cement: Covers root and attaches it to the periodontal ligament.

Tooth and Gum Disease
Dental Caries (Cavities): Demineralization of enamel and dentin by bacterial acids.
Gingivitis: Inflammation of the gums due to plaque and calculus (tartar).
Periodontitis: Advanced gum disease; immune response destroys tissues and bone, increasing risk for tooth loss and systemic diseases.
