Digestive System

Overview of the Digestive Tract

The digestive system is responsible for the breakdown and absorption of nutrients, as well as the elimination of waste. It consists of the digestive tract and accessory organs.

  • Digestive Tract: Mouth (oral cavity), pharynx, esophagus, stomach, small intestine, large intestine

  • Accessory Organs: Teeth, tongue, salivary glands, liver, gallbladder, pancreas

Serous Membranes and Mesenteries

Serous membranes line the abdominal cavity and support the organs within.

  • Peritoneal Membrane: Includes parietal peritoneum (lines cavity), visceral peritoneum (covers organs), and peritoneal cavity (space between).

  • Mesenteries: Double layers of peritoneum that suspend organs and provide pathways for vessels and nerves. Examples: greater omentum, lesser omentum, mesentery proper, mesocolon, falciform ligament.

Tunics (Layers) of the GI Tract

The GI tract wall is composed of four main layers, each with specialized functions.

  • Mucosa: Innermost layer; includes mucosal epithelium, lamina propria (connective tissue), lymphoid tissue (Peyer's patches, MALT), muscularis mucosae, plicae circulares, goblet cells (mucus secretion).

  • Submucosa: Loose areolar connective tissue, blood and lymphatic vessels, submucosal neural plexus (Meissner's plexus).

  • Muscularis Externa: Responsible for peristalsis; consists of longitudinal (outer), circular (inner), and oblique (innermost, stomach only) muscle layers. Contains myenteric plexus (Auerbach).

  • Serosa: Outermost layer; visceral peritoneum or adventitia (dense irregular connective tissue).

Mouth (Oral Cavity)

The mouth is the entry point for food and is involved in mechanical and chemical digestion.

  • Structures: Hard and soft palate, labia, frenula, tongue (intrinsic/extrinsic muscles, lingual tonsils), vestibule, fauces, lingual papillae (filiform, fungiform, circumvallate), uvula, palatine tonsil.

  • Teeth: Types: incisors, canines, premolars, molars. Structure: crown, neck, root, periodontal ligament, cementum, alveolus, gingiva, pulp cavity, root canal, dentin, enamel.

  • Dental Succession: 20 deciduous (baby) teeth, 32 permanent teeth.

  • Salivary Glands: Parotid (Stensen's duct), submandibular (Wharton's duct), sublingual (Rivinus), buccal glands.

Esophagus

The esophagus transports food from the pharynx to the stomach.

  • Layers: Adventitia (outer), muscular layers (upper 1/3 skeletal, middle 1/3 mixed, lower 1/3 smooth muscle).

  • Sphincters: Upper and lower esophageal sphincters (cardiac sphincter).

Stomach

The stomach is a muscular organ that mixes and digests food.

  • Regions: Cardia, fundus, body, pylorus (antrum, canal, orifice, sphincter).

  • Features: Gastric rugae, pits, cells (parietal, chief, mucous, G cells).

  • Blood Supply: Left/right gastric and gastroepiploic arteries/veins.

Small Intestine

The small intestine is the primary site for nutrient absorption.

  • Sections: Duodenum (duodenal papilla, ampulla), jejunum, ileum (ileocecal valve).

  • Features: Mesentery proper, plicae circulares, villi (lacteal, capillary network, goblet cells, microvilli), intestinal crypts (crypts of Lieberkuhn), Peyer's patches.

Large Intestine

The large intestine absorbs water and forms feces.

  • Sections: Cecum (appendix), ascending, transverse, descending, sigmoid colon, rectum, anal canal.

  • Features: Haustra, taeniae coli, epiploic appendages, sphincters (internal/external), pelvic diaphragm.

  • Blood Supply: Superior/inferior mesenteric arteries/veins, internal iliac vessels.

Innervation of the Digestive System

The digestive system is innervated by both sympathetic and parasympathetic fibers.

  • Sympathetic: Superior mesenteric, celiac, hypogastric, inferior mesenteric plexuses.

  • Parasympathetic: Vagus nerve, pelvic splanchnic nerves (S2-4).

  • Somatic: Pudendal nerve (body wall).

Liver

The liver is a vital organ for metabolism, detoxification, and bile production.

  • Lobes: Right, left, caudate, quadrate.

  • Ligaments: Falciform, round (ligamentum teres), coronary.

  • Surfaces: Diaphragmatic (anterior), visceral (posterior).

Gallbladder

The gallbladder stores and concentrates bile.

  • Parts: Fundus, body, neck.

  • Ducts: Cystic duct, common bile duct, hepatopancreatic ampulla (ampulla of Vater), sphincter of Oddi.

Pancreas

The pancreas produces digestive enzymes and hormones.

  • Parts: Head, body, tail.

  • Ducts: Main and accessory pancreatic ducts.

  • Histology: Pancreatic acini (exocrine), islets (endocrine).

  • Blood Supply: Retroperitoneal location.

Urinary System

Kidney Structure

The kidneys filter blood, remove waste, and regulate fluid/electrolyte balance.

  • Coverings: Renal capsule, adipose capsule, renal fascia.

  • Regions: Renal cortex, medulla (pyramids), columns, papilla, sinus.

  • Urine Pathway: Minor/major calyces, anal pelvis.

  • Epithelium: Transitional epithelium (also in ureters, bladder, proximal urethra).

Kidney Blood Supply

Blood is supplied to and drained from the kidneys through a series of arteries and veins.

  • Arteries: Renal, segmental, lobar, interlobar, arcuate, interlobular, afferent arterioles.

  • Capillaries: Glomerular capillaries (glomerulus), efferent arterioles, peritubular capillaries, vasa recta.

  • Veins: Interlobular, arcuate, interlobar, renal veins (no segmental veins).

Innervation of the Kidney

Primarily sympathetic fibers regulate blood flow and renin release; some parasympathetic fibers are present.

  • Renal Plexus: Branch of celiac plexus; controls diameter of renal arteries for autoregulation.

Nephron Structure

The nephron is the functional unit of the kidney, responsible for filtration and urine formation.

  • Renal Corpuscle: Glomerulus (capillary), Bowman's capsule (parietal/visceral layers, podocytes, filtration slits).

  • Renal Tubule: Proximal convoluted tubule, loop of Henle (descending/ascending limbs), distal convoluted tubule.

  • Types: Cortical and juxtamedullary nephrons.

  • Juxtaglomerular Apparatus: Juxtaglomerular cells, macula densa, extraglomerular mesangial cells.

Collecting Tubules and Ducts

Collecting tubules receive filtrate from nephrons and transport urine to the calyces.

  • Pathway: Connecting tubule → collecting duct → papillary duct → minor calyx.

Ureters

Ureters transport urine from the kidneys to the bladder.

  • Layers: Inner mucosa (transitional epithelium), muscular layer (inner longitudinal, outer circular), adventitia.

  • Openings: Ureteral slits in bladder.

Urinary Bladder

The bladder stores urine until excretion.

Urethra

The urethra is the final passage for urine excretion.

  • Sphincters: Internal (smooth muscle), external (skeletal muscle).

  • Female Lining: Transitional → stratified squamous epithelium.

  • Male Lining: Transitional → pseudostratified/stratified columnar → stratified squamous epithelium.

  • Sections (Male): Prostatic, membranous, spongy (penile) urethra.

  • Innervation: Perineal branch of pudendal nerve.

Reproductive System

Male Reproductive System

The male reproductive system produces sperm and delivers it to the female reproductive tract.

  • Testes: Gubernaculum, tunica vaginalis/albuginea, septa, mediastinum, lobules, seminiferous tubules, tubulus rectus, rete testis, efferent ductules, epididymis, interstitial (Leydig) cells.

  • Sperm Structure: Head (23 chromosomes, acrosomal cap), neck (centrioles), middle piece (mitochondria), tail (flagellum).

  • Scrotum: Cavity, perineal raphe, tunica vaginalis, skin, dartos (smooth) and cremaster (skeletal) muscles, nerves, blood supply.

  • Spermatic Cord: Fascia, arteries, veins (pampiniform plexus), vas deferens, nerves, lymphatics.

  • Male Reproductive Tract: Epididymis (head, body, tail), ductus deferens (ampulla), ejaculatory duct, urethra (prostatic, membranous, spongy).

  • Accessory Glands: Seminal vesicles, prostate, bulbourethral (Cowper's) glands.

  • Penis: Root, shaft (corpus spongiosum, corpora cavernosa), glans, prepuce, blood supply, innervation, muscles (ischiocavernosus, bulbospongiosus).

Female Reproductive System

The female reproductive system produces ova, supports fertilization, and houses fetal development.

  • Ovaries: Broad ligament, mesovarium, suspensory/ovarian ligaments, arteries/veins, tunica albuginea, cortex, medulla.

  • Uterine Tubes: Fimbriae, infundibulum, ampulla, isthmus, uterine part, ciliated/nonciliated epithelium, smooth muscle.

  • Mammary Gland: Pectoral fat pad, retromammary space, nipple, areola, lobes/lobules, ducts/sinus, arteries/veins.

Pregnancy and Human Development

Developmental Stages

Human development includes differentiation, prenatal (pre-embryonic, embryonic, fetal), and post-natal stages.

  • Fertilization: Sperm (acrosomal cap, capacitation), oocyte (corona radiata, zona pellucida), pronuclei, amphimixis, zygote, cleavage, morula, blastomeres, blastocyst (blastocoele, trophoblast, inner cell mass).

  • Twins: Monozygotic (identical), fraternal (non-identical).
