Resident Flora of the Gastrointestinal (GI) Tract

The GI tract hosts a complex and diverse community of microorganisms, known as the normal flora, which play essential roles in health and disease. The composition and abundance of these microbes vary by location within the tract.

  • Oral Cavity: Over 700 bacterial species; colonization begins at birth. Dental plaque forms as a biofilm, leading to dental caries if not removed.

  • Stomach: Few bacteria survive due to high acidity. Helicobacter pylori is notable for surviving here and is linked to gastric and peptic ulcers.

  • Small Intestine: Friendlier environment than the stomach, but rapid peristalsis and digestive secretions keep bacterial populations relatively low. Gram-positive bacteria such as Lactobacilli and Enterococcus faecalis are common.

  • Large Intestine: Slow flow allows colonization; about 1,000 species present. Bacteria constitute ~20% of healthy feces.

  • Antibiotics: Can disrupt normal flora, leading to imbalance. Probiotics may help restore healthy flora.

Infections of the Oral Cavity

The oral cavity provides a favorable environment for microbial growth, leading to several common infections:

  • Dental Caries: Tooth decay caused by bacterial metabolism of sugars in dental plaque.

  • Gingivitis: Inflammation of the gums, often reversible with good oral hygiene.

  • Periodontitis: Progression of gingivitis leading to destruction of tooth-supporting structures, tooth loss, and potential systemic infections.

  • Contributing Factors: Smoking, genetics, pregnancy, puberty, stress, medications, and systemic diseases.

  • Necrotizing Ulcerative Gingivitis: Severe form of gum disease with tissue death.

Gastroenteritis

Gastroenteritis is the inflammation of the stomach and intestines, commonly caused by microorganisms or their toxins. It is a leading cause of morbidity and mortality, especially in developing countries.

  • Symptoms: Diarrhea (mild to severe), nausea, vomiting, abdominal cramps, and loss of appetite.

  • Causes: Contaminated food or water, contact with infected individuals, poor hygiene, and contaminated utensils or workspaces.

Bacterial Infections of the GI Tract

Bacterial infections are a major cause of food-borne illness. They may result from ingestion of live bacteria (infection) or preformed toxins (intoxication).

  • Bacterial Infection: Pathogen enters, adheres, and multiplies in the GI tract.

  • Bacterial Intoxication: Illness caused by ingestion of toxins produced by bacteria outside the body.

Common Bacterial Infections

  • Helicobacter pylori (Peptic Ulcer): Causes most gastric and peptic ulcers. Transmission likely via food, water, or close contact. Treated with antibiotics and acid-suppressing drugs.

  • Salmonellosis: Caused by Salmonella spp. from contaminated animal products. Symptoms include diarrhea, fever, and cramps. Prevention involves proper food handling and hygiene.

  • Typhoid Fever: Caused by Salmonella enterica serotype Typhi. Transmitted via contaminated food/water or close contact. Treated with antimicrobials, but resistance is a concern.

  • Paratyphoid Fever: Similar to typhoid but milder; caused by Salmonella enterica (other serotypes).

  • Shigellosis: Bacillary dysentery caused by Shigella spp. Low infective dose; transmitted fecal-orally. Symptoms include bloody diarrhea and cramps.

  • Campylobacteriosis: Caused by Campylobacter jejuni (most common), C. fetus, and C. coli. Leading cause of bacterial diarrhea in the U.S., especially in children and young adults.

  • Escherichia coli Gastroenteritis: Pathogenic strains produce enterotoxins and adhere to intestinal cells. Types include:

    • Enterotoxigenic (ETEC)

    • Enteroinvasive (EIEC)

    • Enteropathogenic (EPEC)

    • Enteroaggregative (EAggEC)

    • Enterohemorrhagic (EHEC)

  • Emerging Infections: Aeromonas hydrophila and Plesiomonas shigelloides from contaminated water or seafood.

  • Yersiniosis: Caused by Yersinia enterocolitica; symptoms vary.

  • Listeriosis: Caused by Listeria monocytogenes; can invade bloodstream and CNS. Highest mortality among food-borne diseases.

Bacterial Intoxications

  • Botulism: Rare, caused by Clostridium botulinum neurotoxin. Symptoms include weakness, blurred vision, and respiratory paralysis. Associated with improperly canned foods.

  • Staphylococcal Intoxication: Staphylococcus aureus produces heat-resistant toxins. Transmitted via contaminated food, often by hand contact.

  • Bacillus Intoxication: Bacillus cereus produces two types of toxins:

    • Diarrheal type: Abdominal pain and watery diarrhea (4–16 hours after ingestion).

    • Emetic type: Nausea and vomiting (30 minutes–24 hours after ingestion).

  • Cholera: Caused by Vibrio cholerae. Characterized by profuse "rice water" diarrhea. Transmitted via contaminated water.

Viral Infections of the GI Tract

Viruses are significant causes of gastroenteritis, especially in children.

  • Rotavirus: Leading cause of infectious diarrhea in infants and children. Vaccines are available.

  • Hepatitis Viruses (A–E): Cause inflammation of the liver.

  • Astroviruses and Caliciviruses: Cause diarrhea, headache, malaise, and nausea.

  • Noroviruses: Cause outbreaks of diarrhea, nausea, and other symptoms; highly contagious.

  • Adenoviruses: Can cause GI and respiratory symptoms.

Fungal Infections (Mycoses) of the GI Tract

Fungal infections are less common but can affect both healthy and immunocompromised individuals.

  • Candidiasis: Overgrowth of Candida species, especially in immunocompromised patients. Oral candidiasis (thrush) is a common manifestation.

  • Aspergillosis: Caused by Aspergillus species. Can lead to acute hepatitis, immunosuppression, and liver cancer.

  • Ergotism: Caused by Claviceps purpurea contaminating grains. Produces alkaloids causing hallucinations, GI upset, gangrene, and burning sensations (St. Anthony’s fire).

Parasitic Infections of the GI Tract

Parasitic infections are caused by protozoans and helminths (worms), with some more common in the U.S. and others prevalent in developing countries.

Protozoan Infections

  • Giardiasis: Giardia intestinalis; water-borne, affects humans and animals.

  • Balantidiasis: Balantidium coli; prevalent in tropics, transmitted fecal-orally.

  • Amebiasis: Entamoeba histolytica; common in underdeveloped countries, waterborne.

  • Cryptosporidiosis: Cryptosporidium; waterborne, common in the U.S.

Helminthic (Worm) Infections

  • Trichinosis: Trichinella spiralis; from undercooked pork.

  • Taeniasis: Taenia saginata (beef), Taenia solium (pork); tapeworms from contaminated meat.

  • Pinworm Infection: Enterobius vermicularis; most common in U.S. children.

  • Ascariasis: Ascaris lumbricoides; most common nematode infection globally.

  • Necatoriasis (Hookworm): Ancylostoma duodenale and Necator americanus; attach to intestinal wall and feed on blood.

  • Trichuriasis: Trichuris trichiura (whipworm); causes large intestine damage, increased drug resistance.

Prevention and Treatment Strategies

  • Hygiene: Handwashing, safe food handling, and clean water are critical for prevention.

  • Vaccination: Available for some viral infections (e.g., rotavirus, hepatitis A and B).

  • Antibiotics/Antimicrobials: Used for bacterial infections, but resistance is a growing problem.

  • Supportive Care: Rehydration and electrolyte replacement are essential, especially in severe diarrhea.

  • Probiotics: May help restore normal flora after antibiotic use.
