Respiratory System Infections
Key Pathogens and Associated Diseases
The respiratory system is susceptible to a variety of infectious agents, including bacteria, viruses, and fungi. Understanding the causative organisms and their clinical manifestations is essential for diagnosis and treatment.
Legionellosis: Caused by Legionella species, typically spread through aerosolized water from sources such as cooling towers, spas, and air conditioning units.
Respiratory Syncytial Virus (RSV): A significant cause of respiratory illness in infants and the elderly. RSV forms syncytia (fused multinucleated cells) and spreads via droplets and contaminated surfaces (fomites).
Streptococcal Pharyngitis (Strep Throat): If untreated, can lead to complications such as scarlet fever, rheumatic fever, and an erythrogenic rash. It does not cause pertussis.
Mycobacterium tuberculosis: The causative agent of tuberculosis. It is slow-growing, has a waxy cell wall, and is resistant to drying and many disinfectants. It does not cause RSV.
Pertussis (Whooping Cough): Caused by Bordetella pertussis. Characterized by paroxysmal coughing. Prevented by the DTaP vaccine, but vaccine immunity is not lifelong. Antivirals are not effective against pertussis.
Influenza: Type A is the most severe. Antigenic drift necessitates new vaccines each year. Begins in the upper respiratory tract and can be fatal. Symptoms include chills, aches, fever, and cough. Dry winter air facilitates transmission.
Sore Throat Etiology: Most commonly caused by viruses rather than bacteria.
Common Cold Viruses: Include Rhinovirus, Coronavirus, and Adenovirus. Epstein-Barr virus is not a common cause.
Fungal Pneumonia in AIDS Patients: Pneumocystis pneumonia (PCP) is the most dangerous.
Healthcare-acquired pneumonia (HAP): Often associated with aspiration or assisted ventilation. It is not rare and can be fatal.

The gastrointestinal (GI) tract is exposed to numerous pathogens, leading to a range of diseases from mild diarrhea to life-threatening conditions.
Gastric Ulcers: Primarily caused by Helicobacter pylori.
Most Common Bacterial Diarrhea in the U.S.: Caused by Campylobacter species.
Clostridioides difficile: Causes pseudomembranous colitis, often after antibiotic use disrupts normal gut flora. Originates from normal biota, not contaminated meats.
Hepatitis: Jaundice is a key symptom. Hepatitis A, B, and C viruses are not genetically related. Hepatitis A does not become chronic and is spread via contaminated food. Hepatitis B can survive in dried blood for several hours.
Pinworm (Enterobius vermicularis): An intestinal worm easily spread via fingers and fomites. Not limited to tropical regions and rarely fatal.
Salmonella enterica: Associated with undercooked poultry and eggs.
Endogenous E. coli: Produces vitamin K in the gut.
Ascaris lumbricoides: Larvae migrate from intestines to lungs, then to the pharynx, are swallowed, and mature in the intestines.
Viral Diarrhea: Vomiting is more common in viral than bacterial GI infections.
Travel-Related Chronic Diarrhea: Most likely caused by Giardia duodenalis.
Mumps: A viral infection of the salivary glands.
Helminth Infection Marker: Eosinophilia (increased eosinophil count in blood).
Dental Plaque: A biofilm on teeth.
Breaking the Fecal-Oral Cycle: Requires water treatment, sanitation, and handwashing.
Hepatitis A: Spread through contaminated food, causes flu-like symptoms, dark urine, and pale stool. Preventable with vaccine.
Acute Food Poisoning Toxins: Caused by exotoxins from Clostridium perfringens, Staphylococcus aureus, and Bacillus cereus. Not caused by Streptococcus mutans.
Cholera: Caused by Vibrio cholerae. Immediate water and electrolyte replacement is critical for treatment.

Eggs are spread on fomites (objects or surfaces).
Eggs are ingested and hatch in the intestine.
Adult worms lay eggs around the anus, causing itching.
Scratching spreads eggs to hands and surfaces, leading to reinfection.

Sexually Transmitted Infections (STIs) and Genitourinary Pathology
The genitourinary (GU) system is affected by a range of pathogens, many of which are sexually transmitted. Understanding their transmission, symptoms, and complications is vital for prevention and management.
Gonorrhea: A sexually transmitted infection (STI) causing painful urination, pelvic inflammatory disease (PID), epididymitis, and infertility. Does not cause chancre.
Prostatitis: Usually caused by endogenous GI flora.
Pelvic Inflammatory Disease (PID): Most commonly caused by Chlamydia infection.
Vaginitis: Most commonly caused by Candida albicans (yeast infection).
Most Common Reported STI in the U.S.: Chlamydia.
Human Papillomavirus (HPV): Can cause cervical, head, and neck cancers. Preventable with vaccine.
Normal Vaginal Flora During Childbearing Years: Dominated by Lactobacillus, which is protective and not indicative of infection.
CDC Monitoring of Gonorrhea: Due to the threat of antibiotic resistance.
Syphilis Staging:
Primary: Painless chancre
Secondary: Fever, hair loss, rash, lymphadenopathy
Tertiary: Neurological and cardiovascular involvement
Role of Lactobacillus: Protective, does not cause yeast infections or PID.
Why Use the Term “STI” Instead of “STD”?
Many infected individuals show no symptoms.
“Disease” implies illness; “infection” is more accurate and inclusive.
Reduces stigma and improves public health communication.
