Overview of Prenatal Development

Prenatal development refers to the process by which a human embryo and later fetus develops during pregnancy, from conception to birth. This period is typically divided into three main stages, each characterized by specific developmental milestones.

  • Germinal Period: The first 2 weeks after conception, involving rapid cell division and implantation of the blastocyst into the uterine wall.

  • Embryonic Period: Spanning from 2 to 8 weeks after conception, this stage is marked by organogenesis, the formation of major organs and structures.

  • Fetal Period: Extending from 2 months after conception until birth, this period involves growth and maturation of tissues and organs.

Example: During the embryonic period, the heart begins to beat and the neural tube (which becomes the brain and spinal cord) forms.

Cellular and Anatomical Changes During Early Development

Fertilization initiates a series of cellular divisions as the zygote travels through the fallopian tube toward the uterus. The following key events occur:

  • Cleavage: Rapid mitotic cell divisions increase the number of cells without increasing the overall size of the embryo.

  • Morula: A solid ball of 16–32 cells forms by day 3–4.

  • Blastocyst: By day 5, a hollow ball of cells develops, which will implant into the uterine wall.

  • Implantation: Occurs around days 6–7, when the blastocyst embeds into the endometrium of the uterus.

Placenta and Fetal Circulation

The placenta is a vital organ that develops during pregnancy, facilitating the exchange of oxygen, nutrients, and waste products between maternal and fetal blood. The umbilical cord contains two arteries and one vein, connecting the fetus to the placenta.

  • Chorion: The outermost fetal membrane, contributing to placenta formation.

  • Amniotic Fluid: Surrounds and cushions the fetus, maintaining a stable temperature and allowing movement.

Example: Oxygenated blood travels from the placenta to the fetus via the umbilical vein, while deoxygenated blood returns to the placenta through the umbilical arteries.

Prenatal Diagnostic Tests

Several diagnostic tests are available to monitor fetal development and detect potential abnormalities during pregnancy:

  • Ultrasound Sonography: Uses high-frequency sound waves to create images of the fetus and assess growth, anatomy, and position.

  • Fetal MRI: Provides detailed images of fetal organs and structures, especially useful for detecting brain and spinal cord abnormalities.

  • Chorionic Villus Sampling (CVS): Involves sampling placental tissue to test for genetic disorders, typically performed between 10–13 weeks of gestation.

  • Amniocentesis: Involves sampling amniotic fluid to test for chromosomal and genetic conditions, usually performed between 15–20 weeks of gestation.

  • Maternal Blood Screening: Measures specific substances in the mother's blood to assess the risk of certain birth defects.

  • Noninvasive Prenatal Diagnosis (NIPD): Analyzes fetal DNA circulating in maternal blood to detect genetic conditions without invasive procedures.
