The Digestive System

Overview

The digestive system is responsible for breaking down food, absorbing nutrients, and eliminating waste. This study guide focuses on the anatomy and physiology of the small and large intestines, as well as the mechanisms of digestion and absorption.

The Small Intestine

Gross Anatomy

The small intestine is the primary site for digestion and absorption of nutrients. It extends from the pyloric sphincter to the ileocecal valve and is subdivided into three regions:

  • Duodenum: ~25 cm long, curves around the head of the pancreas, mostly retroperitoneal.

  • Jejunum: ~2.5 m long, attached posteriorly by mesentery.

  • Ileum: ~3.6 m long, attached posteriorly by mesentery, joins the large intestine at the ileocecal valve.

Blood supply is provided by the superior mesenteric artery, and venous drainage occurs via the hepatic portal vein. The nerve supply includes parasympathetic innervation via the vagus nerve and sympathetic innervation from thoracic splanchnic nerves.

Microscopic Anatomy

The small intestine is highly specialized for absorption, with structural modifications that increase surface area:

  • Circular folds: Permanent folds (~1 cm deep) that force chyme to spiral, increasing absorption time.

  • Villi: Fingerlike projections (~1 mm high) containing capillaries and a lymphatic capillary (lacteal).

  • Microvilli: Cytoplasmic extensions forming the brush border, which contains enzymes for final digestion.

Five main cell types are found in villi and crypts:

  • Enterocytes: Absorptive cells with microvilli, responsible for nutrient and electrolyte absorption.

  • Goblet cells: Mucus-secreting cells.

  • Enteroendocrine cells: Secrete hormones such as CCK and secretin.

  • Paneth cells: Secrete antimicrobial agents (defensins, lysozyme).

  • Stem cells: Continuously divide to renew epithelium.

Mucosa-associated lymphoid tissue (MALT), including Peyer's patches, provides immune protection. Duodenal glands secrete alkaline mucus to neutralize acidic chyme.

Digestive Processes in the Small Intestine

Digestion in the small intestine involves the breakdown of carbohydrates, proteins, and fats. Chyme from the stomach is mixed with bile and pancreatic juice. Brush border enzymes complete digestion.

  • Segmentation: Most common motion after a meal, mixes and moves contents toward the ileocecal valve.

  • Peristalsis: Occurs between meals, propels remnants toward the large intestine.

The Large Intestine

Gross Anatomy

The large intestine absorbs water and electrolytes, forms feces, and eliminates waste. Unique features include teniae coli, haustra, and epiploic appendages. Subdivisions:

  • Cecum: First part, receives chyme from ileum.

  • Appendix: Contains lymphoid tissue, part of immune system.

  • Colon: Ascending, transverse, descending, and sigmoid regions.

  • Rectum: Contains rectal valves.

  • Anal canal: Has internal (smooth muscle) and external (skeletal muscle) sphincters.

Mesenteries anchor the intestines to the posterior abdominal wall. The colon is retroperitoneal except for its transverse and sigmoid parts.

Microscopic Anatomy

The large intestine has a thicker mucosa with abundant goblet cells. The anal canal transitions to stratified squamous epithelium to withstand abrasion. Anal columns and recesses aid in emptying.

Bacterial Flora

Bacterial flora ferment indigestible carbohydrates, synthesize vitamins (B and K), and suppress pathogenic bacteria. The immune system restricts microbes via IgA production.

Digestive Processes in the Large Intestine

Residue remains in the large intestine for 12–24 hours. Water, vitamins, and electrolytes are reclaimed. Major functions include propulsion of feces and defecation.

  • Haustral contractions: Segmenting movements in response to distension.

  • Gastrocolic reflex: Mass movements triggered by food in the stomach.

Defecation Reflex

Mass movements force feces toward the rectum, initiating the spinal defecation reflex. Parasympathetic signals stimulate contraction and relaxation of sphincters. Conscious control allows voluntary relaxation of the external anal sphincter.

Mechanisms of Digestion and Absorption

Enzymatic Hydrolysis

Digestion is a catabolic process that breaks macromolecules into monomers via hydrolysis, using water to break chemical bonds. Enzymes from intrinsic and accessory glands are involved.

Absorption

Absorption moves substances from the gut lumen into the body. Tight junctions ensure molecules pass through epithelial cells. Lipids are absorbed passively; polar molecules require active transport. Most nutrients are absorbed before chyme reaches the ileum.

Only monosaccharides are absorbed. Starch and disaccharides are broken down by salivary and pancreatic amylase, then brush border enzymes. Monosaccharides are cotransported with Na+ and exit via facilitated diffusion.

Proteins are broken down by pepsin in the stomach, then by pancreatic proteases and brush border enzymes in the small intestine. Amino acids are cotransported with Na+ or H+ and exit via facilitated diffusion.

Lipid digestion requires emulsification by bile salts, digestion by pancreatic lipases, micelle formation, diffusion, chylomicron formation, and transport via lymphatic lacteals.

Pancreatic nucleases and brush border enzymes break down nucleic acids into nucleotide monomers, which are absorbed by active transport.

Vitamin Absorption

  • Fat-soluble vitamins (A, D, E, K): Carried by micelles, diffuse into cells.

  • Water-soluble vitamins (C, B): Absorbed by diffusion or transporters.

  • Vitamin B12: Binds with intrinsic factor, absorbed by endocytosis.

Electrolyte Absorption

  • Na+: Coupled with glucose and amino acid absorption.

  • Cl−: Actively transported.

  • K+: Diffuses in response to osmotic gradients.

  • Iron and calcium: Absorbed in duodenum, regulated by need and hormones.

Water Absorption

Most water is absorbed in the small intestine by osmosis, with the remainder absorbed in the large intestine. Water uptake is coupled with solute uptake.

The primitive gut forms from endoderm, with accessory organs budding from mucosa. By week 5, the alimentary canal is a continuous tube from mouth to anus.

Fetal nutrition occurs via placenta; GI tract matures by swallowing amniotic fluid. Newborns have rooting and sucking reflexes. GI activity declines with age, leading to decreased absorption and motility, and increased risk of GI diseases.

Clinical Homeostatic Imbalances

  • Chemotherapy: Targets rapidly dividing cells, causing GI symptoms.

  • Appendicitis: Inflammation of appendix, risk of rupture and peritonitis.

  • Antibiotic-associated diarrhea: C. difficile infection, treated with fecal transplants.

  • Diverticulosis/Diverticulitis: Herniations of mucosa, risk of rupture.

  • Irritable bowel syndrome: Functional disorder, stress-related.

  • Lactose intolerance: Deficiency of lactase, causes diarrhea and gas.

  • Malabsorption: Caused by bile/pancreatic juice delivery issues or celiac disease.

  • Cleft palate/lip, tracheoesophageal fistula, cystic fibrosis: Developmental disorders affecting digestion.
