Overview of the Digestive System
The digestive system is a set of organs responsible for breaking down food into nutrients that can be absorbed and delivered to cells throughout the body. It consists of the alimentary canal (gastrointestinal tract) and accessory organs.
Alimentary canal: Continuous tube including the oral cavity, pharynx, esophagus, stomach, small intestine, and large intestine.
Accessory organs: Assist in digestion but are not part of the canal. Includes teeth, tongue, salivary glands, liver, gallbladder, and pancreas.
Key functions: Ingestion, secretion, propulsion, digestion (mechanical and chemical), absorption, and defecation.
Basic Digestive Functions and Processes
The digestive system performs several essential processes to ensure nutrients are available for the body:
Ingestion: Intake of food and water via the mouth.
Secretion: Release of substances (mucus, enzymes, acids, hormones) by digestive glands.
Propulsion: Movement of food through the canal, mainly via peristalsis (rhythmic contractions of smooth muscle).
Mechanical digestion: Physical breakdown of food (chewing, mixing).
Chemical digestion: Enzymatic breakdown of food into small molecules.
Absorption: Movement of nutrients, water, electrolytes, and vitamins into blood or lymph.
Defecation: Elimination of indigestible materials and metabolic wastes as feces.
Organization of Abdominopelvic Digestive Organs
Digestive organs in the abdominopelvic cavity are supported by serous membranes, blood vessels, and nerves.
Peritoneal membranes: Largest serous membrane, consisting of parietal peritoneum (lines body wall) and visceral peritoneum (covers organs).
Peritoneal cavity: Space between membranes, filled with serous fluid.
Mesenteries: Folds of visceral peritoneum that support organs and house vessels and nerves. Includes greater omentum (four layers, extends from stomach) and lesser omentum (from stomach to liver).
Intraperitoneal vs. retroperitoneal: Organs entirely within the cavity are intraperitoneal; others are retroperitoneal.
Histology of the Alimentary Canal
The alimentary canal has a consistent tissue structure, organized into four layers:
Mucosa: Innermost layer, includes epithelium, lamina propria (connective tissue), and muscularis mucosae (smooth muscle).
Submucosa: Dense irregular connective tissue with blood vessels, lymphatics, glands, and the submucosal plexus (Meissner’s plexus).
Muscularis externa: Two layers of smooth muscle (inner circular, outer longitudinal), regulated by the myenteric plexus (Auerbach’s plexus).
Serosa/Adventitia: Outer layer; serosa (simple squamous epithelium and loose connective tissue) for intraperitoneal organs, adventitia (dense irregular connective tissue) for retroperitoneal organs.
Regulation of Motility by the Nervous and Endocrine Systems
Motility (movement) in the digestive system is regulated by both nervous and endocrine systems:
Autonomic nervous system (ANS): Sympathetic inhibits, parasympathetic stimulates motility (long reflexes).
Enteric nervous system (ENS): Local, self-contained branch of ANS; regulates short reflexes within the digestive tract.
Endocrine regulation: Hormones (often paracrines) produced by digestive organs stimulate or inhibit motility.
The Oral Cavity, Pharynx, and Esophagus
Structure and Function of the Oral Cavity
The oral cavity is the entry point for digestion, where ingestion, secretion, mechanical and chemical digestion, and propulsion occur.
Boundaries: Cheeks (buccinator muscles), lips (orbicularis oris), gums (gingivae), palate (hard and soft), and tongue.
Vestibule: Space between teeth/gums and lips/cheeks.
Palate: Hard palate (anterior, rough for mechanical digestion), soft palate (posterior, arch-shaped, includes uvula).
The Teeth and Mastication
Teeth are essential for mechanical digestion, breaking food into smaller pieces to increase surface area for enzymes.
Types of teeth: Incisors (cutting), canines (tearing), premolars and molars (grinding).
Primary dentition: 20 deciduous teeth (baby teeth).
Secondary dentition: 32 permanent teeth (including wisdom teeth).
Tooth structure: Crown (enamel), root (cementum), dentin (elastic mineralized tissue), pulp (connective tissue, blood vessels, nerves).
The Tongue
The tongue is a muscular organ covered with epithelium, involved in mechanical digestion, taste, and propulsion.
Papillae: Filiform (mechanical), fungiform, circumvallate, foliate (contain taste buds).
Muscle groups: Extrinsic (position), intrinsic (shape and size).
Salivary Glands
Three pairs of salivary glands secrete saliva, which aids in digestion and oral health.
Parotid: Serous cells, secrete water and enzymes.
Submandibular: Mostly serous, some mucous cells.
Sublingual: Mostly mucous cells.
Saliva components: Water, electrolytes, mucus, salivary amylase (carbohydrate digestion), lysozyme (antibacterial), IgA (immune), bicarbonate (neutralizes acid).
Functions: Moistening, lubricating, cleansing, mechanical and chemical digestion, taste stimulation.
Pharynx and Esophagus
The pharynx and esophagus are responsible for propulsion, moving the bolus from the oral cavity to the stomach.
Pharynx: Oropharynx and laryngopharynx are part of the alimentary canal; lined with protective epithelium; houses tonsils for immune defense.
Esophagus: Muscular tube (10 inches), lined with protective epithelium; contains upper and lower (gastroesophageal) sphincters; primary function is propulsion via peristalsis.
Swallowing (Deglutition)
Swallowing is a coordinated process involving the oral cavity, pharynx, and esophagus, divided into three phases:
Voluntary phase: Tongue pushes bolus toward oropharynx (conscious control).
Pharyngeal phase: Bolus enters oropharynx; soft palate and epiglottis seal off nasopharynx and larynx; involuntary muscle contraction (swallowing reflex).
Esophageal phase: Peristaltic waves move bolus down esophagus; regulated by ENS.
The Stomach
Gross Anatomy of the Stomach
The stomach is a J-shaped organ in the upper left abdomen, with five anatomical regions:
Cardia: Entry point from esophagus.
Fundus: Dome-shaped top.
Body: Largest portion.
Pyloric antrum: Inferior portion.
Pylorus: Terminal portion, contains pyloric sphincter (controls flow to duodenum).
Rugae: Folds allowing expansion.
Histology of the Stomach
The stomach wall has four layers, with unique features:
Muscularis externa: Three layers (inner oblique, middle circular, outer longitudinal) for churning.

Gastric phase: Triggered by food in stomach; distension and protein presence stimulate acid secretion.
Intestinal phase: Triggered by chyme entering duodenum; brief stimulation, then inhibition via hormones (secretin, GIP).
Motility: Stomach receives, churns, and empties food. Churning is controlled by gastric pacemaker cells; peristalsis propels chyme to pylorus.
Gastric emptying: Controlled by duodenal feedback (stretch, pH, lipid content); hormones (secretin, CCK, GIP) delay emptying to allow proper mixing and neutralization.
