Overview of Spinal Nerve Structure

The spinal nerves are critical components of the peripheral nervous system (PNS), connecting the central nervous system (CNS) to limbs and organs. Each spinal nerve is formed by the union of dorsal and ventral roots and branches into rami that serve different regions of the body.

  • Root: The initial segments of a spinal nerve, consisting of the dorsal (sensory) and ventral (motor) roots emerging from the spinal cord.

  • Nerve: The mixed spinal nerve formed by the fusion of the dorsal and ventral roots, carrying both sensory and motor fibers.

  • Ramus (plural: rami): After exiting the vertebral column, each spinal nerve splits into dorsal and ventral rami, which innervate the posterior and anterior body regions, respectively.

  • Plexus: A network of intersecting nerves formed by the ventral rami of adjacent spinal nerves, allowing for redistribution of nerve fibers to various body regions.

  • Ganglion: A cluster of neuron cell bodies located outside the CNS, such as the dorsal root ganglion, which contains sensory neuron cell bodies.

The Four Spinal Nerve Plexuses

The ventral rami of spinal nerves (except in the thoracic region) form four major plexuses, each giving rise to nerves that innervate specific regions of the body.

  • Cervical Plexus (C1–C5): Supplies the skin and muscles of the neck, ear, back of the head, and shoulders. Example: The phrenic nerve (C3–C5) innervates the diaphragm, essential for breathing.

  • Brachial Plexus (C5–T1): Innervates the shoulders, arms, and hands. Example: The radial nerve controls muscles of the posterior arm and forearm.

  • Lumbar Plexus (L1–L4): Supplies the anterior and medial thigh, as well as parts of the abdominal wall. Example: The femoral nerve innervates the quadriceps muscle group.

  • Sacral Plexus (L4–S4): Innervates the pelvis, buttocks, genitals, thighs, calves, and feet. Example: The sciatic nerve is the largest nerve in the body, serving the lower limb.

Additional info: The thoracic nerves (T2–T12) do not form a plexus but become the intercostal nerves, which innervate the intercostal muscles and skin of the thorax.

Functions of Major Spinal Nerves

Major spinal nerves emerging from the plexuses have distinct motor and sensory functions, enabling voluntary movement and sensory perception in the limbs and trunk.

  • Phrenic Nerve: Controls the diaphragm, essential for respiration.

  • Radial Nerve: Extends the wrist and fingers; provides sensation to part of the hand.

  • Median Nerve: Flexes the wrist and fingers; supplies sensation to the lateral palm.

  • Ulnar Nerve: Controls fine movements of the hand; supplies sensation to the medial hand.

  • Femoral Nerve: Extends the knee; provides sensation to the anterior thigh and medial leg.

  • Sciatic Nerve: Innervates the hamstrings and all muscles below the knee; provides sensation to most of the leg and foot.

Additional info: Damage to these nerves can result in loss of motor function, sensory deficits, or neuropathic pain in their respective regions.
