Heart Circuits
Pulmonary and Systemic Circuits
The heart functions as a dual pump, circulating blood through two main circuits: the pulmonary and systemic circuits. Each circuit serves a distinct purpose in oxygenating blood and distributing it throughout the body.
Pulmonary Circuit: Right ventricle (RV) → Pulmonary trunk/arteries → Lungs → Pulmonary veins → Left atrium (LA). Function: Carries deoxygenated blood to the lungs for gas exchange and returns oxygenated blood to the heart.
Systemic Circuit: Left ventricle (LV) → Aorta → Body → Superior/Inferior Vena Cava (SVC/IVC) → Right atrium (RA). Function: Distributes oxygenated blood to the body and returns deoxygenated blood to the heart.

Pericardium Structure
The pericardium is a protective sac surrounding the heart, consisting of several layers:
Fibrous pericardium: Outermost tough layer.
Parietal serous pericardium: Lines the inner surface of the fibrous pericardium.
Pericardial cavity: Space containing serous fluid to reduce friction.
Visceral serous pericardium (Epicardium): Directly covers the heart.
Heart Wall Layers
The heart wall is composed of three distinct layers:
Epicardium: Outer layer (same as visceral pericardium).
Myocardium: Middle, thickest layer; composed of cardiac muscle tissue responsible for contraction.
Endocardium: Inner layer lining the heart chambers and valves.
Cardiac Muscle Characteristics
Striated and branched fibers.
Intercalated discs: Specialized junctions containing desmosomes (mechanical strength) and gap junctions (electrical connectivity).

Orientation and Chambers
The heart is positioned with its apex pointing left and inferior, and its base superior and posterior. It contains four chambers:
Right Atrium (RA)
Right Ventricle (RV)
Left Atrium (LA)
Left Ventricle (LV): Has the thickest wall due to high pressure required for systemic circulation.
Heart Valves
Tricuspid valve: Between RA and RV.
Pulmonary valve: Between RV and pulmonary trunk.
Mitral (Bicuspid) valve: Between LA and LV.
Aortic valve: Between LV and aorta.

SVC/IVC → RA → Tricuspid valve → RV → Pulmonary valve → Lungs → LA → Mitral valve → LV → Aortic valve → Aorta
Coronary Vessels
Coronary Arteries and Veins
The heart receives its own blood supply via the coronary circulation.
Arteries: Right coronary, Left coronary, Anterior interventricular (LAD), Circumflex.
Veins: Great, Middle, Small cardiac veins → Coronary sinus → RA.

Electrical Pathway of the Heart
The heart's rhythm is controlled by a specialized conducting system:
Order: SA node → AV node → AV bundle (Bundle of His) → Right/Left bundle branches → Purkinje fibers
SA node: Primary pacemaker (60–100 bpm), initiates heartbeat.
Cardiac Cycle
Atrial systole: Contraction of atria.
Ventricular systole: Contraction of ventricles.
Complete diastole: Relaxation of all chambers.

ANS Regulation of Heart Function
Sympathetic: Increases heart rate and contractility via norepinephrine (NE) acting on β1 receptors.

Definition: Abnormal valve sounds caused by turbulent blood flow.

Example: Blood Flow Pathway
Deoxygenated blood enters the right atrium via the SVC/IVC, passes through the tricuspid valve to the right ventricle, then through the pulmonary valve to the lungs. Oxygenated blood returns to the left atrium, passes through the mitral valve to the left ventricle, and is pumped through the aortic valve to the systemic circulation.
