Introduction to the Digestive System

The digestive system is responsible for the breakdown and absorption of nutrients from food, as well as the elimination of waste products. It consists of the digestive tract (also called the gastrointestinal tract or alimentary canal) and accessory organs that assist in digestion.

  • Digestive Tract: Extends from the mouth to the anus (~9 meters).

  • Accessory Organs: Include salivary glands, teeth, tongue, liver, gallbladder, and pancreas.

Major Functions of the Digestive System

The digestive system performs several essential functions to maintain homeostasis and provide energy for the body.

  • Ingestion: Intake of food through the mouth.

  • Mechanical Digestion: Physical breakdown of food by chewing (mouth), churning (stomach), and segmentation (small intestine).

  • Chemical Digestion: Enzymatic breakdown of food into small molecules for absorption.

  • Secretion: Release of digestive substances (enzymes, acids, mucus) into the lumen of the tract.

  • Absorption: Movement of nutrients, water, and ions into blood or lymph.

  • Excretion: Removal of waste products via defecation.

Layers of the Digestive Tract

The wall of the digestive tract is composed of four main layers, each with specific functions.

  • Mucosa: Innermost layer; contains epithelium, connective tissue, and muscularis mucosae. Specialized for secretion and absorption.

  • Submucosa: Dense connective tissue with blood vessels, lymphatics, nerves, and glands.

  • Muscularis Externa: Two (or three in the stomach) layers of smooth muscle responsible for peristalsis and segmentation.

  • Adventitia/Serosa: Outermost layer; adventitia (retroperitoneal organs) is connective tissue, serosa (intraperitoneal organs) is a serous membrane.

Mucosa Details

The mucosa varies by location and function.

  • Epithelium: Nonkeratinized stratified squamous (mouth, pharynx, esophagus, anus); simple columnar (stomach, small intestine, large intestine).

  • Muscularis Mucosae: Thin layer of smooth muscle; forms folds to increase surface area.

  • Connective Tissue: Contains blood/lymphatic vessels, nerve plexus, glands, and lymphatic tissue.

Muscularis Externa and Peristalsis

  • Muscularis Externa: Two sheets (circular and longitudinal); stomach has a third (oblique) layer.

  • Peristalsis: Rhythmic contractions that propel food through the tract.

Serosa and Adventitia

  • Serosa: Serous membrane covering intraperitoneal organs.

  • Adventitia: Connective tissue covering retroperitoneal organs.

Mesenteries and Peritoneum

Mesenteries are double layers of peritoneum that attach organs to the cavity wall, store fat, and hold blood vessels/nerves.

  • Parietal Peritoneum: Lines the abdominal wall.

  • Visceral Peritoneum: Covers the organs.

  • Peritoneal Cavity: Contains peritoneal fluid to decrease friction.

Components of the Digestive Tract

Oral Cavity

The oral cavity is the entry point for food and is involved in mechanical and chemical digestion.

  • Functions: Mechanical digestion (chewing), lubrication, initiation of carbohydrate digestion (salivary amylase), formation of bolus.

Pharynx

The pharynx connects the oral cavity to the esophagus and is involved in swallowing.

  • Regions: Oropharynx and laryngopharynx are part of the digestive system.

  • Function: Moves bolus to esophagus.

The esophagus is a muscular tube that transports food from the pharynx to the stomach.

  • Location: Posterior to trachea; extends from laryngopharynx to stomach.

  • Function: Uses peristalsis to move solids and liquids.

  • Sphincter: Cardiac/gastroesophageal sphincter regulates entry to the stomach.

Stomach

The stomach is a J-shaped organ that stores and digests food.

  • Regions: Cardia, fundus, body, pyloric region.

  • Functions: Storage, mechanical digestion (churning), chemical digestion of proteins (pepsin), production of intrinsic factor.

  • Pyloric Sphincter: Controls movement of chyme into the duodenum.

Small Intestine

The small intestine is the primary site for digestion and absorption.

  • Regions: Duodenum (closest to stomach), jejunum, ileum.

  • Histology: Simple columnar epithelium, villi, microvilli, goblet cells, intestinal glands.

  • Functions: Chemical digestion (lipids via bile, carbohydrates and proteins via pancreatic enzymes), absorption of nutrients, water, and ions.

  • Hepatic Portal System: Nutrients absorbed are transported to the liver.

Large Intestine

The large intestine absorbs water and vitamins, compacts feces, and stores waste.

  • Regions: Cecum, colon (ascending, transverse, descending, sigmoid), rectum, anal canal.

  • Cecum: Blind pouch below ileum; appendix attached.

  • Colon: Last 20 cm is the rectum; no serosa.

  • Anal Canal: Forms internal (involuntary) and external (voluntary) anal sphincters.

  • Teniae Coli: Muscle bands that maintain tone and form haustra.

  • Functions: Reabsorbs water, vitamins, bile salts, compacts feces, stores fecal material.

Salivary Glands, Teeth, and Tongue

  • Salivary Glands: Produce saliva containing enzymes for carbohydrate digestion.

  • Teeth: Mechanical breakdown of food.

  • Tongue: Assists in mixing food and swallowing.

Liver, Gallbladder, and Pancreas

  • Liver: Produces bile for emulsification of fats.

  • Gallbladder: Stores and releases bile.

  • Pancreas: Produces digestive enzymes and bicarbonate.
