Functional Anatomy of the Central Nervous System (CNS)

Overview of the CNS

The central nervous system (CNS) consists of the brain and spinal cord, serving as the main control center for processing and integrating information. It is responsible for conscious thought, memory, sensory processing, and somatic motor movements.

  • Brain: Divided into several regions, each with specialized functions.

  • Spinal Cord: Acts as a neural pathway between the brain and peripheral nervous system (PNS), and is the integration spot for spinal reflexes.

Cerebrum

The cerebrum is the largest portion of the brain, responsible for higher-order functions such as conscious thought, memory, and voluntary motor control. It is divided into two hemispheres and four lobes.

  • Cerebral Cortex: The superficial layer composed of gray matter; deep to this is white matter.

  • Gyri: Highly folded ridges that increase surface area.

  • Sulci: Shallow depressions between gyri.

  • Fissures: Deeper grooves separating major regions.

  • Corpus Callosum: Thick white matter connecting the two hemispheres, allowing communication.

Cerebral Lobes

Each hemisphere is divided into four lobes, each with distinct functions:

  • Frontal Lobe: Responsible for voluntary motor functions, planning, and speech production.

  • Parietal Lobe: Processes sensory information and spatial orientation.

  • Temporal Lobe: Involved in auditory processing and memory.

  • Occipital Lobe: Dedicated to visual processing.

Functional Areas of the Cerebral Cortex

  • Primary Motor Cortex: Located anterior to the central sulcus; sends voluntary motor information.

  • Primary Somatosensory Cortex: Posterior to the central sulcus; receives sensory information.

  • Wernicke’s Area: Integrates sensory information and memory for language comprehension.

  • Broca’s Area: Regulates breathing and vocalization for speech.

  • Association Areas: Integrate sensory and motor cortexes.

  • Homunculus: A visual representation of the body mapped onto the cortex, showing the distribution of motor and sensory functions.

Hemispheric Lateralization

Although the left and right cerebral hemispheres appear identical, they have specialized functions. Each hemisphere receives sensory information from and sends motor information to the opposite side of the body, a process known as "crossing over." This occurs in the brainstem and spinal cord.

  • Left Hemisphere: Language, mathematical calculation, and logic.

  • Right Hemisphere: Spatial visualization, analysis, and creativity.

Cerebellum

The cerebellum contains about 50% of the brain’s neurons and is essential for coordinating skeletal muscle movements, interpreting proprioceptive input, and sequencing motor activities. Its myelinated structure, called the Arbor vitae ("tree of life"), is visible in cross-section.

Diencephalon

The diencephalon acts as a relay center connecting the cerebral hemispheres to the rest of the CNS. It consists of:

  • Thalamus: Relay and processing center for sensory information; filters sensory input.

  • Epithalamus: Includes the pineal gland, which secretes melatonin for sleep/wake cycles.

  • Hypothalamus: Regulates autonomic functions, emotions, body temperature, hunger/thirst, daily cycles, and acts as the "master regulator" of the endocrine system.

Brainstem

The brainstem is composed of the midbrain, pons, and medulla oblongata. It is crucial for relaying information and regulating vital autonomic functions.

  • Midbrain: Contains reflex centers, substantia nigra (produces dopamine), and reticular formation for consciousness.

  • Pons: Multiple relay stations, motor control, and regulation of breathing rhythm.

  • Medulla Oblongata: Lowest part of the brainstem; regulates heart rate, blood pressure, respiratory rate, and reflexes such as vomiting, hiccupping, swallowing, and sneezing.

CSF and Ventricles

Cerebrospinal Fluid (CSF) and Ventricular System

CSF is a clear fluid that cushions the brain, provides buoyancy, and transports nutrients, chemicals, and waste. The ventricles are interconnected chambers within the brain filled with CSF.

  • Lateral Ventricles: Located in each cerebral hemisphere.

  • Third Ventricle: Located in the diencephalon.

  • Fourth Ventricle: Located between the brainstem and cerebellum.

  • Central Canal: Extends into the spinal cord.

Cranial Meninges

The cranial meninges are three layers of specialized membranes that protect the brain and spinal cord, stabilize the CNS, and facilitate the flow of cerebrospinal fluid.

  • Dura Mater: Tough, outermost layer.

  • Arachnoid Mater: Middle layer, web-like structure.

  • Pia Mater: Innermost layer, closely adheres to the brain and spinal cord.

Spinal Cord Anatomy and Function

The spinal cord is a neural pathway between the brain and PNS, integrating spinal reflexes. It starts at the foramen magnum and ends at about the L2 vertebra, after which the terminal region is called the "Cauda Equina." The spinal cord gives rise to 31 pairs of spinal nerves.

  • Segments: Cervical, thoracic, lumbar, sacral, and coccygeal.

  • Spinal Nerves: Each segment gives rise to a pair of spinal nerves.

The spinal cord has a central, butterfly-shaped gray matter core surrounded by white matter columns. The central canal contains CSF. The dorsal root and ventral root of the spinal nerve serve sensory and motor functions, respectively.

  • Gray Matter: Contains neuron cell bodies; involved in processing information.

  • White Matter: Contains myelinated axons; responsible for transmitting signals.

  • Dorsal Root: Carries sensory information to the CNS.

  • Dorsal Root Ganglion: Contains cell bodies of sensory neurons.

  • Ventral Root: Carries motor information from the CNS to muscles and glands.
