Symbiosis

Symbiosis refers to the close association between two different biological species. These relationships can vary in their effects on the organisms involved.

  • Commensalism: One organism benefits while the other is neither helped nor harmed. Example: Staphylococcus epidermidis living on human skin.

  • Mutualism: Both organisms benefit from the relationship. Example: Gut bacteria such as Escherichia coli synthesize vitamins for the host and receive nutrients.

  • Parasitism: One organism (the parasite) benefits at the expense of the other (the host). Example: Pathogenic bacteria causing disease in humans.

  • Opportunism: Normally harmless organisms become pathogenic under certain conditions, such as immunocompromised states.

Normal flora: The collection of microorganisms that reside on or within the human body without causing disease under normal circumstances.

Health vs. Disease - A Balancing Act

The state of health or disease is determined by the dynamic interaction between the microbe and the host.

  • Factors of Microbe:

    • Virulence: The degree of pathogenicity; more virulent microbes cause more severe disease.

    • Dose: The number of organisms required to cause disease.

    • Portal of Entry: The route by which a pathogen enters the host (e.g., respiratory tract, gastrointestinal tract).

    • Synergy: Interaction between multiple microbes that can enhance disease severity.

  • Factors of Host:

    • Immunocompetence: The ability of the host to mount an effective immune response.

    • Immunocompromise: Reduced immune function, increasing susceptibility to infection.

Disease vs. Health: The outcome depends on the balance between microbial factors and host defenses.

Transmission of Disease

Diseases can be transmitted through various mechanisms, classified as direct or indirect.

  • Direct Transmission: Immediate transfer of pathogens from person to person.

    • Respiratory droplets: Pathogens spread via droplets expelled during coughing or sneezing.

  • Indirect Transmission: Pathogens are transferred via intermediaries.

    • Vehicles: Non-living carriers such as water, food, or air.

    • Fomites: Inanimate objects that can harbor infectious agents (e.g., doorknobs, medical equipment).

    • Vectors: Living organisms (usually arthropods) that transmit pathogens (e.g., mosquitoes, ticks).

Zoonoses: Diseases transmitted from animals to humans. Reservoirs: Sources of infectious agents in the environment. Carriers: Individuals who harbor pathogens without showing symptoms, but can transmit them to others.

The Course of Disease: Stages

The progression of infectious disease typically follows several stages:

  • Incubation: Time between exposure and onset of symptoms. Example: Influenza incubation is 1-4 days.

  • Prodromal: Early, mild symptoms appear.

  • Acute ("Illness"): Full-blown symptoms; disease is at its peak.

  • Decline: Symptoms begin to subside as the immune response overcomes the pathogen.

  • Convalescence: Recovery phase; tissues heal and function returns to normal.

Course of Disease: Two Variants

  • Acute Disease: Rapid onset and short duration; often reaches a crisis point before resolution. Example: Influenza.

  • Chronic Disease: Slow onset and long duration; symptoms may persist or recur. Example: Tuberculosis.

Timing of Diseases

  • Primary Diseases: Initial infection in a healthy host.

  • Secondary Diseases: Subsequent infections that occur due to weakened host defenses.

  • Sequelae: Long-term or permanent consequences following an infection (e.g., rheumatic heart disease after streptococcal infection).

  • Nosocomial Infections (HAI): Infections acquired in healthcare settings. Three common examples: Urinary tract infections, surgical site infections, pneumonia.

Special considerations: Hospital environments increase risk due to high pathogen exposure and vulnerable patients.

Location of Diseases

  • Local Infection: Confined to a specific area or tissue.

  • Systemic Infection: Spread throughout the body via blood or lymph.

    • Septicemia: Presence of pathogens in the blood causing systemic illness.

    • Bacteremia: Bacteria in the blood.

    • Viremia: Viruses in the blood.

    • Fungemia: Fungi in the blood.

  • Focal Infection: Localized infection that spreads to other parts of the body.

Pattern of Diseases in Populations (Epidemiology)

Epidemiology studies the distribution and determinants of diseases in populations.

  • Endemic: Disease constantly present in a population or region. Example: Malaria in certain tropical areas.

  • Epidemic: Sudden increase in disease cases above normal levels in a population. Example: Influenza outbreak.

  • Pandemic: Epidemic that spreads across countries or continents. Example: COVID-19.
