Epidemiology
Definition and Scope
Epidemiology is the study of the distribution and determinants of health-related states or events (including disease) in specified human populations. It is a foundational science for public health, focusing on patterns, causes, and effects of health and disease conditions.
Pathogen characteristics (such as mode of transmission, virulence, and environmental stability) are crucial for understanding how diseases spread and persist.
Epidemiologists trace the spread of disease to identify its source and mode of transmission.
Key Epidemiological Terms
Disease prevalence: The total number of cases (both new and existing) of a disease in a population at a given time.
Disease incidence: The number of new cases of a disease that occur in a specified population during a defined time period.
Patterns of Disease Occurrence
Epidemic: A sudden increase in the number of cases of a disease above what is normally expected in that population in that area.
Pandemic: An epidemic that has spread over several countries or continents, usually affecting a large number of people.
Endemic disease: A disease that is constantly present, usually at low frequency, in a population or region.
Sporadic: Occasional cases of a disease occurring at irregular intervals and in scattered locations.
Outbreak: The occurrence of cases of disease in excess of what would normally be expected in a defined community, geographical area, or season.
Carriers, Mortality, and Morbidity
Carriers: Individuals who harbor a pathogen without showing symptoms but can transmit the pathogen to others.
Mortality: The number of deaths caused by a disease in a population during a specific period.
Morbidity: The state of being diseased or the incidence of illness in a population.
Disease Reservoirs and Zoonoses
Reservoirs
Reservoir: Any person, animal, plant, soil, or substance in which an infectious agent normally lives and multiplies.
Reservoirs can be human or non-human (e.g., animals, environment).
In some cases, the reservoir is asymptomatic, making control more difficult.
Carriers and Zoonoses
Carrier: An individual who is infected with a pathogen but does not show symptoms (i.e., asymptomatic) and can be temporary or chronic.
Example: "Typhoid Mary" was a famous carrier of Salmonella typhi.
Zoonosis: A disease that can be transmitted from animals to humans.
Pathogen Life Cycle and Eradication
A pathogen that depends solely on the human host for completion of its life cycle may be more easily eradicated than those which survive or spend part of their life cycle in non-human reservoirs.

Transmission Overview
Transmission of infectious diseases is a central focus of epidemiology, involving the mechanisms by which pathogens move from one host to another.
Different pathogens have different transmission mechanisms depending on their biology and environmental factors.
Stages of Successful Transmission
Escape from the host (source of infection)
Travel to a new host (via air, water, vectors, etc.)
Entry into the new host and establishment of infection
Transmission Mechanisms
Direct transmission: Pathogen is transferred directly from one host to another (e.g., person-to-person contact, droplets).
Indirect transmission: Pathogen is transferred via an intermediate object or organism (e.g., contaminated surfaces, food, water, vectors).
Vector: A living organism (often an arthropod such as a mosquito or tick) that transmits an infectious agent from one host to another.
Epidemics and Host Community Factors
Epidemics can be influenced by factors such as population density, immunity levels, and social behavior.
Factors to consider in the host community include:
Density of susceptible and immune individuals
Population movement and mixing
Hygiene and sanitation practices
Access to healthcare and vaccination

Efforts to control infectious diseases may target:

Emerging and Re-emerging Infectious Diseases
Emerging infectious diseases are those that have recently appeared within a population or whose incidence or geographic range is rapidly increasing.
Re-emerging diseases are those that were once under control but are again becoming health problems.

Transmission Pathways
Airborne Transmission
Air is generally not a favorable environment for microbial survival, but some microorganisms survive better than others under dry, desiccating conditions during transport.
Most respiratory pathogens are adapted to survive and transmit via airborne routes.
Examples of Bacteria Responsible for Respiratory Disease
Streptococcus pneumoniae
Mycobacterium tuberculosis
Bordetella pertussis
Legionella pneumophila
Corynebacterium diphtheriae
Many viral diseases also result from airborne transmission, such as influenza, measles, and chickenpox.
Foodborne Disease
Foodborne diseases can be considered as outbreaks (epidemics) or sporadic cases.
Food Poisoning (Intoxication)
Staphylococcus aureus produces several enterotoxins which get released into the surrounding food.
Clostridium perfringens produces an enterotoxin which causes diarrhea and abdominal cramps.
Clostridium botulinum produces the most severe and often fatal form of food poisoning known (botulism).
Foodborne Infection
The pathogen is growing within the host after introduction via contaminated food.
Examples of bacteria causing foodborne infection include: Salmonella, Shigella, Escherichia coli, Campylobacter, Listeria monocytogenes.
Waterborne Disease
Waterborne transmission occurs through ingestion of contaminated water, which may contain pathogens or their toxins.
Usually considered to be fecal-oral; the number of organisms required to cause disease depends on the pathogen and host susceptibility.
Example: Vibrio cholerae (causative agent of cholera) is a waterborne pathogen that can cause severe diarrheal disease and dehydration.
Infections Transmitted by Drinking Water
Most important bacteria include Salmonella typhi and Vibrio cholerae, Shigella.
Legionella pneumophila is a newer concern for the drinking water industry.

Cryptosporidium parvum (cryptosporidiosis)
