Respiratory System Infections

Key Pathogens and Diseases

The respiratory tract is susceptible to a variety of infectious agents, including bacteria, viruses, and fungi. Understanding the causative organisms and their transmission is essential for diagnosis and prevention.

  • Legionellosis: Caused by Legionella pneumophila, transmitted via aerosolized water from sources such as cooling towers, spas, and air conditioning units.

  • Respiratory Syncytial Virus (RSV): Significant in infants and the elderly. Forms syncytia (large multinucleated fused cells) via viral fusion proteins. Spread by droplets and fomites.

  • Untreated Streptococcal Pharyngitis Complications: Can lead to scarlet fever, rheumatic fever, and erythrogenic rash. Does not cause pertussis.

  • Mycobacterium tuberculosis: Slow-growing bacterium with a waxy cell wall, resistant to drying and disinfectants. Does not cause RSV.

  • Whooping Cough (Pertussis): Caused by Bordetella pertussis. Characterized by paroxysmal coughing. Prevented by DTaP vaccine, but immunity is not lifelong. Antivirals are not used for treatment.

  • Influenza: Type A is most severe. Undergoes antigenic drift, necessitating new yearly vaccines. Begins in the upper respiratory tract and can be fatal. Symptoms include chills, aches, fever, and cough. Dry winter air facilitates spread.

  • Sore Throat Etiology: Most commonly caused by viruses.

  • Common Cold Viruses: Rhinovirus, Coronavirus, Adenovirus (not Epstein-Barr virus).

  • Fungal Pneumonia in AIDS Patients: Most dangerous is Pneumocystis pneumonia (PCP).

  • Healthcare-acquired pneumonia (HAP): Often associated with aspiration or assisted ventilation. Not rare and can be fatal.

Syncytial Cells

Syncytial cells are large, multinucleated cells formed by the fusion of individual cells, typically due to viral infections such as RSV. Viral fusion proteins mediate this process, resulting in cell–cell merging.

  • Definition: Large multinucleated fused cells.

  • Cause: Viral infections (e.g., RSV).

  • Mechanism: Viral fusion proteins cause cell–cell merging.

  • Example: RSV infection in infants.

Gastrointestinal System Infections

Stomach and Intestinal Diseases

The gastrointestinal tract is affected by a range of pathogens, including bacteria, viruses, and helminths. Disease manifestations vary from mild diarrhea to severe systemic illness.

  • Gastric Ulcers: Caused by Helicobacter pylori.

  • Most Common Bacterial Diarrhea in U.S.: Campylobacter species.

  • Clostridioides difficile: Causes pseudomembranous colitis, often following antibiotic disruption of normal flora. Originates from normal biota, not contaminated meats.

  • Hepatitis Symptoms: Jaundice is a key sign.

  • Pinworm: Intestinal worm easily contaminates fingers and fomites. Not limited to tropical regions and rarely fatal.

  • Salmonella enterica: Associated with undercooked poultry and eggs.

  • Endogenous E. coli: Produces vitamin K in the gut.

  • Hepatitis Viruses A–C: Not genetically related.

  • Ascaris lumbricoides: Larvae migrate intestines → lungs → pharynx → swallowed → mature in intestines.

  • Viral Diarrhea Indicator: Vomiting is more common in viral than bacterial illness.

  • Hepatitis that Does Not Become Chronic: Hepatitis A.

  • Travel-Related Chronic Diarrhea: Most likely caused by Giardia duodenalis.

  • Viral Infection of Salivary Glands: Mumps.

  • Helminth Infection Marker: Eosinophilia.

  • Biofilm on Teeth: Plaque.

  • Breaking the Fecal-Oral Cycle: Requires water treatment, sanitation, and handwashing.

  • Hepatitis A Characteristics: Spread through contaminated food. Causes flu-like symptoms, dark urine, pale stool. Preventable with vaccine.

  • Acute Food Poisoning Toxins: Clostridium perfringens exotoxin, Staphylococcus aureus exotoxin, Bacillus cereus exotoxin (not Streptococcus mutans).

  • Hepatitis B Exposure Risk: Can survive several hours in dried blood.

  • Cholera Treatment: Immediate water and electrolyte replacement.

Helminth Life Cycle Example: Pinworm

Helminths are parasitic worms with complex life cycles. Pinworm infection is common and easily transmitted.

  • Eggs spread on fomites.

  • Ingestion: Eggs are ingested and hatch in the intestine.

  • Adult worms: Lay eggs around the anus.

  • Transmission: Scratching spreads eggs to hands and surfaces, leading to reinfection.

  • Example: Enterobius vermicularis (pinworm).

Genitourinary System Infections

Sexually Transmitted Infections and GU Pathology

The genitourinary system is affected by a variety of pathogens, many of which are sexually transmitted. Understanding their clinical manifestations and prevention is crucial.

  • Gonorrhea: STI causing painful urination, PID, epididymitis, and infertility. Does not cause chancre.

  • Prostatitis: Usually caused by endogenous GI flora.

  • Pelvic Inflammatory Disease (PID): Most commonly caused by Chlamydia.

  • Most Common Cause of Vaginitis: Candida albicans.

  • Most Common Reported STI in U.S.: Chlamydia.

  • HPV: Can cause cervical, head, and neck cancers. Preventable with vaccine.

  • Normal Vaginal Flora During Childbearing Years: Dominated by Lactobacillus.

  • CDC Monitoring of Gonorrhea: Due to antibiotic resistance threat.

  • Syphilis Staging:

    • Primary: Painless chancre.

    • Secondary: Fever, hair loss, rash, lymphadenopathy.

    • Tertiary: Neurological/cardiovascular involvement.

  • Role of Lactobacillus: Protective, not indicative of infection. Does not cause yeast infections or PID.

Why Say “STI” Instead of “STD”?

The term "STI" (Sexually Transmitted Infection) is preferred over "STD" (Sexually Transmitted Disease) because many infected individuals show no symptoms. "Disease" implies illness, while "infection" is more accurate and reduces stigma.

  • Asymptomatic cases: Many individuals are infected but do not show symptoms.

  • Terminology: "Infection" is more accurate than "disease."

  • Stigma reduction: Using "STI" improves communication and reduces stigma.
