Principle of the Urease Test
The urease test is a biochemical assay used to detect the ability of microorganisms to hydrolyze urea using the enzyme urease. This reaction is important for identifying certain bacteria, especially those associated with urinary tract infections.
Urea is a diamide of carbonic acid. It is hydrolyzed by urease, resulting in the release of ammonia (NH3) and carbon dioxide (CO2).
Many bacteria that infect the urinary tract possess urease, enabling them to split urea in the presence of water.
The released ammonia increases the pH of the medium, making it alkaline. This pH change is detected by a color indicator (usually phenol red), which shifts from its original orange-yellow color to bright pink in alkaline conditions.

Test Procedure and Interpretation
Organisms are inoculated onto a urea-containing medium (e.g., urea agar or broth).
Incubation is typically at 35-37°C.
Results are interpreted based on color change:
Positive result: Bright pink color (alkaline pH due to ammonia production).
Negative result: No color change or remains yellow/orange.
Slow positive: Weak or delayed pink color, indicating slow urease activity.
Examples and Applications
Rapid urease producers: Proteus species (e.g., Proteus vulgaris) may yield positive results within 1-2 hours.
Slow urease producers: Some species (e.g., Klebsiella) may require 3 or more days for a positive result.
In routine diagnostic laboratories, the urease test result is read within 24 hours.
